Abstract

Atherogenic dyslipidemia is a common metabolic abnormality associated with type 2 diabetes mellitus (T2DM). The Atherogenic Index of Plasma (AIP), calculated as the logarithm of the triglyceride-to-high-density lipoprotein cholesterol (TG/HDL-C) ratio, has been proposed as a more comprehensive indicator of atherosclerotic risk than conventional lipid parameters. This study aimed to compare lipid profiles and AIP values between patients with T2DM and non-diabetic individuals in the general population of Malang City, Indonesia. This analytical observational study employed a cross-sectional design involving 77 participants, including 30 patients with T2DM and 47 non-diabetic individuals. The analyzed variables included serum total cholesterol, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides (TG), and AIP, which was calculated using the formula log₁₀(TG/HDL-C). Comparisons between groups were performed using the independent-samples t-test, with statistical significance defined as p < 0.05. Patients with T2DM had significantly higher levels of total cholesterol (208.40 ± 43.78 vs. 187.98 ± 40.81 mg/dL; p = 0.045), LDL-C (127.14 ± 32.35 vs. 105.09 ± 29.02 mg/dL; p = 0.004), triglycerides (226.30 ± 138.60 vs. 124.30 ± 59.47 mg/dL; p < 0.001), and AIP (0.73 ± 0.27 vs. 0.44 ± 0.22; p < 0.001) than non-diabetic individuals. Although HDL-C levels were lower in the T2DM group (37.81 ± 8.95 vs. 42.05 ± 9.89 mg/dL), the difference was not statistically significant (p = 0.056). Patients with T2DM exhibited a more atherogenic lipid profile and significantly higher AIP values than non-diabetic individuals. These findings support the potential use of AIP as a complementary biomarker to conventional lipid profile assessment for identifying individuals at increased risk of atherosclerosis and cardiovascular disease among patients with T2DM.
